How Safety Protocol Failed El Faro

The search for El Faro and her crew has been called off. Many questions remain as to how and why a ship was sailing into a hurricane and who exactly made the decisions that cost 33 men their lives.

As always in these tragic situations, the questions will be asked, safety procedures will be pored over and there will be an attempt to apportion blame.

Will all this be enough to prevent another accident at sea in the future?
